Instructional Model Switch Request Form

Instructional Model Switch Request Form
Posted on 04/27/2021

Greetings TeamLCPS Families:

Last month on March 15, our school division provided an important update that provided details for our instructional plans for the 2021-2022 school year. In our March 15 update, we asked all LCPS families to select one of the following instructional models for their child for the 2021-2022 school year:

- The Face-To-Face Model: In this model, we will provide students with five days of in-school instruction in our school buildings each week. Health and safety guidelines will continue to be utilized.
- LCPS Virtual Academy: In this model, students receive five days of instruction per week through a fully virtual environment. Courses could potentially be taught by an LCPS teacher, through a fully online program facilitated by (but not taught by) LCPS staff, or a combination of both, depending on the classes that are offered.

In our original March 15 update, we also included the link to a survey where all LCPS parents/guardians had the opportunity to select the instruction model they would prefer for their child for the 2021-2022 school year. The deadline to complete that survey was Friday, March 26th.

Since that time, we have received and/or finalized a selection for all LCPS students enrolled for the 2021-2022 school year, and we thank all of you who took the time to complete this important survey. At the same time, we also want to provide you with an opportunity to alter your original decision if you would like. Accordingly, during the week of May 3rd to May 7th, we will be providing you with this opportunity.

If you would like to alter the original selection you made for your child’s instructional model for the 2021-2022 school year, please complete one of the following steps by May 7th:

Visit our division website ( and complete the Instructional Model Survey form on the home page; or - Contact your child’s school directly and request to switch learning models.

It’s important to note that if you are satisfied with the original choice you made for your child’s instructional model for the 2021-2022 school year, no action is necessary. However, we want to provide this opportunity to any families who may need it.

Once again, thank you TeamLCPS for your continued support and active participation in your child’s educational experience. This information is very valuable as we continue to create plans and procedures for the upcoming school year!  Together, we will SET THE STANDARD for excellence!
